We will start with a 30 minute Case Review call to get acquainted, review you records, set goals and orient my efforts to meet your specific needs. You will then complete several forms for me to review. These include a short intake form, your specific conditions, nutrition and diet habits, personal goals and lifestyle constraints. I will use this information to prepare for your consulting hour.
We will then schedule a 60 minute private session (in-office or by telehealth) where I will explore more of your diet and lifestyle patterns and the daily challenges you face. I will then describe changes and interventions you can implement right away and a plan of action to help you adopt these changes (such as foods to add or avoid, factors to monitor and items to adjust to keep you on track).
In addition, I am available for 7 days after you receive your report to answer your email questions and ensure a successful implementation of the recommendations listed in your report.
